A= almost always            S= sometimes            N=almost never

1. The interpreter arrives on time or slightly before class.
    AA= almost always   S= late once or twice   N= late more than 3 times
2. The interpreter signed/interpreted clearly and I understood the lecture.
3. The interpreter finger spelled words clearly and I understood the lecture.
4. The interpreter signed in my preferred mode - used ASL, PSE, Oral, etc.
5. I was able to participate in class activities by sharing my opinion, asking questions,
    ect. because the interpreter was able to sign/voice my comments.
6. The interpreter seem to act and handle themselves as a professional.
7. The interpreter was able  to voice my questions and comments asking me to repeat
     only a few times.
8. The interpreter used good facial, body, or mouth movements that helped me
     understand the lecture.
9. The interpreter seemed to understand the lecture, class vocabulary, and as well
     prepared for the class.
10. The interpreter dressed appropriately/neatly.
11. I felt the interpreter kept information confidential.
